Sports Physicals
ATTENTION ALL SPORT ATHLETES
Physicals are for all students entering 7th - 12th grade for the 2024-2025 school year. Physical sessions are for ALL student athletes participating in a FALL, WINTER, or SPRING sport.
***NEW*** Student athletes who have a sport physical dated prior to 10/04/2024 must complete a recertification form (PIAA section 7: Recertification by a Parent/Guardian) for Winter sports

The final FREE physical session for the 2024-2025 school year will be held in the East High Main Gym on Wednesday, November 13, 2024 from 3-6 pm.
The physical sign-up has been CLOSED. Students who attempt to walk in from 3-6 pm are NOT guaranteed an appointment. Students who had signed up online will take priority.
This session is for students who attend EMS, SMS, EHS, LMS, CDMS and CDHS in 7th -12th grade interested in participating in a Winter and/or Spring sport during the 2024-2025 school year. Students who attend affiliated Cyber, Home School or DCTS can attend this session if they are eligible to participate in sports at CDSD
